Dear parents,


Due to an elevated amount of new consultation requests, the pediatric pulmonology registration form is currently closed.

The registration form will reopen once consultations become available.

Important: In order to be seen by a specialist, a referral from a family doctor or dentist is required.

Conditions include chronic cough, asthma, recurrent pneumonia, sleep apnea, insomnia, sleep terrors, sleepwalking, etc.

To register for a pediatric respirology appointment, please fill out the form below and attach the consultation request from your physician/pediatrician. Make sure the consultation request is clear, legible and completed.

Please note: Completing and submitting the referral form does not guarantee that your child will be assigned to a pediatric pulmonologist/respirologist. Patients are triaged based on the urgency of the medical attention needed. 

You will be contacted by a member of our clinic if your child is assigned to a pediatric pulmonologist/respirologist.
