Pediatric dermatology is for children and adolescents from 3 months to 18 years of age who suffer from different skin diseases or issues.

Bloom is a speciality clinic staffed only with pediatricians. Pediatrics is a speciality medicine focused on treating children with a persisting medical issue. Our long-term pediatric care is primarily focused on healing children with serious or chronic health issues. Children without health problems will not be accepted, except siblings of children already followed by a doctor in our clinic, provided that your pediatrician has agreed to follow them.
In order to be admissible, all new patients require a referral from a family doctor or other healthcare professional for a pediatric consult.
If you have a pediatric referral for your child, please complete the form below.
Please note: completing and submitting the referral form does not guarantee that your child will be assigned to a pediatrician. Patients are triaged based on the urgency of the medical attention needed. You will be contacted by a member of our clinic if your is assigned to a pediatrician.
Make sure the consultation request is clear, legible and completed.

If you wish to schedule an appointment for a child under 14 years old, please create a MYLE account using your details (those of the parent or legal guardian).
Afterward, please send us an email at info@cmme.ca to notify us of the account creation, including:
1- your name
2- your child's name
3- their RAMQ number
Please note that it is not necessary to create a portal to schedule appointments for minor emergencies and dermatology.
